Abstract
패혈증은 감염에 대한 전신성 염증 반응을 동반하는 치명적인 질환으로, 적절한 항생제 투여가 1시간 지연될 때마다 환자의 생존율이 7~10%씩 급격히 감소한다. 임상적 긴급성에도 불구하고, 현 표준 진단법인 배양 검사는 미생물 증식에만 72시간 이상이 소요되며 병원균 동정 및 항생제 감수성 검사(AST)까지 총 2~6일이 소요된다. 이러한 진단 지연은 광범위 항생제 오남용과 다제내성균 확산을 초래하여 환자 사망률을 낮추는 데 심각한 걸림돌이 되고 있다. 이를 해결하기 위해, 대용량 혈액 샘플로부터 극저농도의 병원균을 무표지(Label-free) 상태로 고속 연속 분리할 수 있는 전처리 기술 개발이 시급하다.
기존 관성 미세유체 플랫폼들은 실제 임상 샘플 적용 시 명확한 공학적 한계를 나타냈다. 나선형 채널이나 트렌치 웰 구조를 적용한 시스템들은 혈액 희석 배수를 낮출 경우 밀집된 혈구 세포 간의 물리적 간섭으로 인해 분리 순도가 약 70% 수준으로 크게 저하되었다. 더욱이 박테리아의 형태학적 구조에 따라 포획 성능 편차가 극심하여, 구균(Cocci) 형태의 병원균에 대해서는 회수율이 심각하게 급감함으로써 진단 신뢰성을 보장하기 어려웠다.
본 연구에서는 이러한 한계를 극복하기 위해, 전혈로부터 ~1μm 크기의 박테리아를 형태에 관계없이 고속·고효율로 분리 및 회수할 수 있는 새로운 이층 구조의 다중 턴 데크레센도 삼각형(Decrescendo Triangle, DT) 미세유체 디바이스를 제안한다. 제안된 디바이스는 고-종횡비 직사각형 하부 메인 채널(1st layer; 폭 400μm, 높이 20μm)과 삼각형 구조물이 주기적으로 배열된 상부 채널(2nd layer; 높이 40μm)이 통합되어 총 높이 60μm 및 약 6.7의 단면 종횡비를 구성한다. 이 고-종횡비 구조는 입자가 여러 궤적으로 분산되는 것을 억제하고 상하 2개의 평형 궤적으로만 집중되도록 유도할 뿐만 아니라, 수력학적 저항을 낮추어 대용량 시료의 고속 처리를 가능하게 한다. 상부 삼각형 구조물의 축 방향 길이는 출구 방향으로 갈수록 수축하는 채널 폭의 감소율을 정밀하게 반영하여, 입구 측 800μm에서 출구 측 100μm까지 비선형 감쇠 함수를 따라 점진적으로 축소되도록 설계되었다.
본 DT 채널의 핵심 작동 원리는 유동의 대칭성을 깨뜨리고 채널 레이놀즈 수(Rec)와 입자 레이놀즈 수(Rep)의 관계를 정밀하게 제어하는 유체역학적 메커니즘에 기반한다. 일정한 폭을 가진 기존 직선형 채널에서는 삼각형 구조물의 폭이 넓어지는 구간마다 유속이 감소하여 Rep가 집속 임계치 미만(Rep 〈 1.0)으로 하락하며 입자를 평형 궤도에 고정하는 유체역학적 힘을 상실하게 된다. 반대로, 단순히 주입 유량을 높일 경우 수축부에서 Rec가 상한 임계치(Rec 〉 100)를 돌파하여 유속 평탄화 현상이 발생하고, 이로 인해 유동장 자체가 붕괴되는 상충 관계가 존재한다. 제안된 데크레센도 기하학 구조는 내부 유속을 국소적으로 가속하는 동시에 채널의 수력학적 직경(Dh)을 점진적으로 감소시켜 이 문제를 본질적으로 해결한다. Rec 수식에서는 Dh의 감소가 분자의 유속 증가분을 상쇄하여 Rec를 50 미만의 안정적인 수치로 통제하는 반면, Rep 수식에서는 유속 증가와 직경 감소가 강력한 시너지를 일으켜 하류로 갈수록 Rep만을 임계점인 1.0 이상으로 기하급수적으로 증폭시킨다.
유동 해석 결과, 이러한 메커니즘은 채널 전반부에서 10-8 N 수준의 강력한 구조적 이차 유동 항력을 통해 입자를 거시적으로 유도하고, 출구 측 수축부에서 Rep 〉 1.0을 달성하여 입자를 벽면 평형 단일 궤적에 강력하게 밀착시키는 이상적인 이단계 집속 메커니즘을 구현함을 검증하였다. 단면 벡터장 정량 비교 분석을 통해, 구조적 장치가 없는 순수 데크레센도 채널은 이차 유동 항력이 10-9 N 수준으로 급감하여 입자를 초기 유선에서 이탈시킬 구동력이 현저히 부족한 반면, 본 DT 채널은 관성 양력과 이차 항력 조건을 동시에 극대화함을 입증하였다. 특히, 이 유체역학적 거동은 입자 크기에 극도로 민감하게 작용한다. 직경이 4~8μm 이상으로 큰 혈액 세포는 증폭된 Rep에 의해 강력한 관성 양력을 받아 타겟 벽면으로 명확히 집속되는 반면, 크기가 ~1μm 수준인 미세 박테리아는 관성 효과를 거의 받지 않고 구조물이 유도하는 이차 유동의 크기와 방향 벡터에 전적으로 지배되어 포커싱되지 않고 메인 유선을 따라 그대로 흘러간다. 이 거동 차이와 사전 설계된 1:4의 출구 유량 저항비를 결합하여, 혈구 세포는 폐기구(Outlet 2)로 유도하고 박테리아는 회수구(Outlet 1)를 통해 연속적으로 수집하는 메커니즘을 완성하였다.
본 미세유체 디바이스는 실리콘 웨이퍼 위에 SU-8 3025 감광액을 이용한 다층 포토리소그래피 공정으로 몰드를 제작한 후, PDMS와 경화제를 9:1 무게비로 혼합 및 주조하고 75°C 오븐에서 90분간 경화한 뒤 박리하여 산소 플라즈마 처리를 통해 슬라이드 글라스 기판과 영구 접합하는 소프트 리소그래피 공정으로 제작되었다. 강체인 1.0μm, 8.0μm, 16.0μm 형광 마이크로비드를 이용한 직선형 DT 채널 기초 검증 결과, 최적 유량 40 mL/h 조건에서 8.0μm 및 16.0μm 입자가 각각 99.78%와 99.89%의 완벽한 분리 효율로 Outlet 2로 배출됨을 확인하였다. 그러나 이를 실제 전혈 샘플(1x PBS 및 0.5% Tween 20 기반 1% HCT)에 적용했을 때는 혈구 세포 고유의 유연성(Deformability)과 혈액의 복잡한 점도 특성으로 인해 분리 효율이 77.84%로 크게 저하되는 현상이 관찰되었다.
이러한 전임상적 격차를 극복하기 위해, 제한된 칩 면적 내에서 세포와 구조물 간의 상호작용 횟수를 극대화할 수 있는 다중 턴 구조(Turn 1, Turn 2, Turn 3)를 최종 고안하였다. 채널에 곡선 구간이 도입되면 곡률에 따른 원심력의 영향으로 횡단면에 상하 대칭의 이중 소용돌이인 딘 유동(Dean flow)이 발생한다. 이로 인해 생성되는 딘 항력은 기존 데크레센도 삼각형 구조의 벽면 포커싱 능력을 보조하여 혈구 세포를 내측 벽면으로 강하게 밀착시키는 강력한 시너지 효과를 창출한다. 턴 수 누적에 따른 유체 저항 증가로 최적 구동 유량은 다소 감소하였으나, 유량별 성능 해석을 통해 우수한 분리 효율과 처리량 간의 최적의 균형을 제공하는 Turn 2 모델(최적 유량 20 mL/h)을 최종 진단 플랫폼으로 선정하였다.
최종 최적화된 Turn 2 채널을 활용하여 패혈증 케이스 스터디를 수행하였다. 1% HCT 혈액 샘플에 그람 음성균인 대장균(E. coli)과 그람 양성균인 황색포도상구균(S. aureus)을 저농도부터 고농도 스펙트럼(101, 102, 103 CFU/mL)까지 스파이킹하여 평가한 결과, 모든 농도 구간에서 80% 이상의 우수한 박테리아 회수율을 안정적으로 달성하였다. 특히, 미세유체 시스템에서 비특이적 흡착과 데드 볼륨 손실로 인해 포획이 극도로 까다로운 101 CFU/mL의 극저농도 환경에서도 성능 저하 없이 높은 회수율을 온전히 유지하는 견고함을 보여주었다. 나아가, 균의 형태학적 특성(간균 및 구균)에 관계없이 동등하게 우수한 분리 성능을 기록함으로써, 균종에 따라 성능이 급감하던 기존 기술의 치명적인 한계를 성공적으로 해결하였다.
결론적으로, 본 연구에서 개발된 다중 턴 데크레센도 삼각형 미세유체 플랫폼은 패혈증 진단에 소요되는 진단 전처리 시간을 획기적으로 단축하여 환자의 생명을 살릴 수 있는 실효성 있는 무표지 전처리 기술임을 명확히 입증한다. 향후 후속 연구에서는 채널의 병렬화(Parallelization) 및 기하학적 다중화를 통해 전혈 기준의 절대적 처리량을 극대화할 것이며, 분리·농축된 샘플을 하위 분자 진단(PCR) 및 자동화 항생제 감수성 검사(AST) 칩과 즉각적으로 연계하여, 샘플 투입부터 최종 병원균 동정까지 원스톱으로 이루어지는 '통합형 현장 진단(POC) 플랫폼'으로 발전시켜 나갈 것이다.|The Sepsis is a critical global health crisis characterized by a life-threatening systemic inflammatory response, where a patient's survival rate precipitates by 7–10% for each hour of delay in appropriate antimicrobial administration. Despite its clinical urgency, the current diagnostic gold standard relies heavily on blood culture protocols that fundamentally require over 72 hours for micro-organism cultivation, followed by an additional 2 to 6 days for downstream pathogen identification and antimicrobial susceptibility testing (AST). This protracted diagnostic turnaround time inevitably prompts the empirical misuse of broad-spectrum antibiotics, exacerbating the emergence of multi-drug resistant strains and driving a vicious cycle of elevated mortality. To circumvent this clinicopathological bottleneck, rapid, robust, and label-free sample preparation technologies capable of continuously isolating trace-level pathogens directly from blood volumes are urgently required.
Although conventional microfluidic platforms exploiting inertial or elasto-inertial hydrodynamic forces have demonstrated potential for cell separation, they consistently encounter critical engineering limitations when subjected to real-world clinical boundaries. Prior systems utilizing spiral channels or sequential trench wells exhibit a severe degradation in separation purity (often dropping to ~70%) under reduced sample dilution factors due to steric hindrance and hydrodynamic interactions among densely packed blood cells. More critically, existing platforms suffer from acute performance discrepancies dictated by bacterial morphology; while bacilli-shaped bacteria are moderately recovered, the isolation yield for spherical cocci strains plummets drastically, undermining diagnostic reliability.
To fundamentally resolve these structural and morphological limitations, this study proposes a novel two-layer Multiple-Turn Decrescendo Triangle (DT) microfluidic device optimized for the high-speed, high-efficiency, and morphology-independent isolation of microscale bacteria (~1μm) from whole blood. The geometric architecture of the device comprises a high-aspect-ratio rectangular bottom main channel (1st layer; 400μm width, 20μm height) integrated with an upper asymmetric channel (2nd layer; 40μm height) containing periodically arranged triangular structures, achieving a total channel height of 60μm and a cross-sectional aspect ratio of approximately 6.7. This high-aspect-ratio design effectively suppresses particle dispersion across multiple regional quadrants, confining cell trajectories to distinct equilibrium paths while drastically reducing internal hydrodynamic resistance to facilitate large-volume processing. To maximize the lateral migration force, the axial length of the upper triangular features is designed to systematically scale down from 800μm at the inlet to 100μm at the outlet, adhering to a pre-defined non-linear decay function that precisely mirrors the contraction rate of the narrowing channel width.
The core working principle of the DT channel relies on a precisely orchestrated hydrodynamic mechanism that breaks fluid symmetry and balances the channel Reynolds number (Rec) and the particle Reynolds number (Rep). In conventional straight channels with uniform cross-sections, fluid velocity decreases as the asymmetric triangular features widen, causing Rep to drop below the critical threshold (Rep 〈 1.0$), which destabilizes the trajectory-holding force. Conversely, simply increasing the volumetric flow rate to compensate for this loss pushes Rec beyond its upper boundary (Rec 〉 100) at the contraction zones, causing a velocity flattening phenomenon that dissipates the shear gradient and collapses the flow field. The proposed decrescendo geometry inherently mitigates this hydrodynamic trade-off; according to the continuity equation, the progressively contracting channel width locally accelerates the internal fluid velocity while decreasing the hydraulic diameter (Dh). In the Rec governing equation, the reduction in Dh offsets the increment of maximum velocity in the numerator, maintaining Rec at a stable regime below 50. Simultaneously, in the Rep equation, where velocity resides in the numerator and Dh in the denominator, the concurrent acceleration and diameter reduction produce a profound synergy that exponentially amplifies Rep beyond the critical threshold of 1.0.
Numerical flow simulations verified that this mechanism creates an ideal biphasic focusing paradigm: the anterior section macroscopically translates particles via structural secondary drag forces on the order of 10-8 N, while the contracting posterior section drives Rep 〉 1.0, definitively locking the target cells into a single, tightly focused equilibrium trajectory near the wall. Quantitative vector field comparative analysis demonstrated that while a pure decrescendo channel lacks the structural mechanisms to induce adequate secondary flows—resulting in an insufficient lateral drag force on the order of 10-9 N—the proposed DT configuration simultaneously maximizes both the inertial lift and secondary drag conditions. Crucially, this hydrodynamic alignment exhibits an extreme dependence on particle size. Relatively large blood cells (4–8μm) exploit the amplified Rep to experience an overriding inertial lift force that overcomes secondary flow drag, pinning them to the target wall. In stark contrast, minute pathogens (~1μm) possess negligible Rep and remain exclusively governed by the magnitude and directional vectors of the continuous secondary flows, following the main streamlines without focusing. Combined with a pre-designed outlet fluidic resistance ratio of 1:4, blood cells are cleanly driven into the waste outlet (Outlet 2) while bacteria are continuously harvested via the collection outlet (Outlet 1).
The microfluidic device was fabricated via a multi-layered negative photolithography and soft lithography process using SU-8 3025 photoresist on a silicon wafer, followed by polydimethylsiloxane (PDMS) casting (9:1 weight ratio), degassing, thermal curing at 75°C for 90 minutes, and permanent bonding to a glass slide via oxygen plasma treatment. Baseline validation using rigid fluorescent microbeads (1.0μm, 8.0μm, and 16.0μm) in a straight DT configuration at an optimized flow rate of 40 mL/h yielded near-perfect separation efficiencies of 99.78% and 99.89% for the 8.0μm and 16.0μm particles into Outlet 2, respectively. However, applying the straight architecture to real clinical whole blood samples (diluted to 1% hematocrit in 1x PBS with 0.5% Tween 20) led to a severe performance degradation, dropping the blood cell depletion efficiency to 77.84% due to the intrinsic deformability of blood cells and complex sample viscosity.
To bridge this translation gap, the architecture was optimized into a multiple-turn structure (Turn 1, Turn 2, and Turn 3) to increase the interaction frequency between cells and geometric features within a compact footprint. Bending the channel introduces a centrifugal curvature effect, inducing counter-rotating, vertically symmetrical double vortices known as Dean flow. The resulting lateral Dean drag force works synergistically with the decrescendo triangle features, robustly guiding entering blood cells toward the inner wall. While increasing the turn numbers escalated fluidic resistance and shifted the optimal operating flow rates downward, the Turn 2 channel established an exceptional balance between fluidic stability and processing speed. Under an optimized throughput of 20 mL/h, the Turn 2 model achieved a stable and superior blood cell separation efficiency.
In clinical simulation case studies for sepsis, the Turn 2 device was evaluated by spiking Gram-negative Escherichia coli (E. coli) and Gram-positive Staphylococcus aureus (S. aureus) into 1% HCT blood across a wide concentration spectrum (101, 102, and 103 CFU/mL). The platform successfully achieved a robust bacterial recovery rate exceeding 80% across all testing regimes. Remarkably, high recovery performance was fully preserved even at the ultra-low concentration of 101 CFU/mL, completely bypassing the non-specific wall adsorption and system dead-volume losses that typically plague rare-cell isolation. Furthermore, the device demonstrated uniform efficacy regardless of the structural and morphological profiles of the bacteria (bacilli vs. cocci), successfully resolving the fatal limitation of conventional sorting mechanisms.
In conclusion, the multiple-turn decrescendo triangle microfluidic platform developed in this work offers a highly viable, practical, and instrument-free sample preparation technology. By drastically compressing the time-consuming pre-analytical phase of sepsis diagnostics, this platform holds immense clinical potential to secure the critical therapeutic golden hour. Future development will focus on scaling up the absolute whole-blood throughput via channel parallelization and geometric multiplexing, alongside direct downstream integration with molecular diagnostics (PCR) and automated AST chips to realize a fully automated, one-stop point-of-care (POC) diagnostic system.
